Pre-Procedure Checklist

  Prep Instructions
Please refer to prep handout – Questions, contact your MD office.

  Preop Clearance
If directed, please schedule immediately once you know your procedure date.

  Lab Work
If directed, please schedule immediately once you know your procedure date.

  Recreational Drug Use
Please refrain from the use of ALL recreational drugs within 24 hours of the day of your procedure. This includes all forms of marijuana. Failure to do so will result in the cancellation of your procedure.

  Appointment Confirmation Text Messages
Please note you will receive appointment reminders via text:

• 13 days prior to procedure
• 3 days prior to procedure (with time of arrival)
• 24 hours prior to procedure

  Copays/Deductibles
Notification of out-of-pocket responsibility will be received within 3-5 days of your procedure.

  Preop Nurse Interview
Please expect a call from an RN 24-48 hours prior to your visit.

  Preop Anesthesia Airway Evaluation
Schedule only if directed.

  Designated Escort REQUIRED Day of Procedure
You will need an escort (18 years or older) responsible for your care to sign you out and drive you home following your procedure. Your escort will be expected to return to pick you up within 1.5 hours of your scheduled time of arrival and/or be asked to stay on the premises until you are ready for discharge. (An Uber or taxi is NOT an escort.)

  Post op Check-in
Expect a text on the business day following your visit. We will be checking in to see how you are feeling.

  Patient Satisfaction Survey
Please let us know about your overall experience. A survey will be forwarded via text/email following your procedure.
